Name Change

The Passport Agency has just recently updated it's Passport Amendment Policy. If you are looking to get a Passport Name Change, please read this update.
  1. Complete and Submit the Passport Book application online (Apply for a Passport Book). Print the DS-11 form that is created for your at the end.
  2. You are required to submit the original copy of the legal document verifying the name change, such as a marriage certificate or a court order.
  3. Submit these documents, along with your U.S. Passport, and a Visa Network Order Form to:

Expedited service takes approximately ten business days to complete. For a Rush, Will Call service, a flight itinerary or Business letter stating the urgency of this immediate international travel is required. This process generally takes about three days to complete.
